Guinea Pig Corneal Disease: Diagnostic Workup
Bottom line
A cloudy or painful guinea-pig eye should be localized to cornea, anterior chamber, lens, globe, adnexa, or retrobulbar tissue before treatment. Guinea pigs can show limited reflex tearing despite significant corneal disease, so mild discharge does not equal mild pathology. Stain early, assess depth and globe integrity, avoid corticosteroids until epithelial ulceration and infection are excluded, and investigate systemic or dental contributors when the lesion is atypical or recurrent.
Triage and focused examination
Assess appetite, fecal output, hydration, weight, pain, trauma, and respiratory status before prolonged restraint. Globe rupture, deep or melting ulcer, proptosis, uncontrolled hemorrhage, rapidly progressive opacity, or inability to close the lids requires immediate stabilization and ophthalmic planning. Provide species-appropriate analgesia and nutritional support when ocular pain suppresses intake.
Examine both eyes in consistent illumination. Evaluate menace or navigation behavior, dazzle and pupillary responses where interpretable, eyelids, cilia, conjunctiva, third eyelid, globe position, corneal surface, anterior chamber, iris, lens, and fundus. Published examinations in guinea pigs have identified foreign bodies, trichiasis, conjunctival inflammation, corneal fibrosis, and vascularization.[1]
Localize the opacity
Use magnification and fluorescein stain before topical medication. Record ulcer dimensions, depth, margin, stromal edema, infiltrate, vascularization, anterior-chamber flare, hypopyon, pupil shape, and lens opacity. Seidel testing is indicated when perforation is possible. Rebound tonometry can help separate pressure-associated disease but must be interpreted with corneal thickness, restraint, pain, and device limitations.
Corneal disease in rodents spans foreign-body injury, exposure, dystrophy or degeneration, tear-film disease, ulcerative keratitis, secondary infection, and nutritional or husbandry contributions.[2] Guinea-pig corneas may be relatively insensitive, with little reflex tearing, so absence of dramatic blepharospasm should not reassure.[2]
Sampling and deeper investigation
Perform cytology and culture before antimicrobials for stromal infiltrate, malacia, deep ulcer, recurrence, or treatment failure. Sample ulcer margins rather than only necrotic centers. Consider ultrasound when the posterior segment is obscured and globe integrity permits it.
A complete oral and dental examination plus skull imaging is appropriate with exophthalmos, nasolacrimal disease, facial swelling, malocclusion, weight loss, or recurrence. CT is preferred when tooth roots, orbit, bone, or a foreign body must be mapped. Distinguish a clear cornea with lens opacity from corneal clouding; cataract and corneal ulcer require different plans.
Treatment and monitoring logic
Remove superficial foreign material only with adequate visualization, analgesia, and control of movement. Select topical therapy from ulcer depth, cytology, culture, and anterior-segment inflammation. Avoid topical anesthetic dispensing and avoid corticosteroids in an ulcerated or infected cornea. Address exposure, lid disease, dental disease, vitamin C adequacy, and environmental hazards without substituting supplements for lesion-directed therapy.
Recheck deep, infected, or progressive ulcers early and document stain retention, depth, dimensions, vascularization, pain, intake, and weight. The ocular-pain workup broadens the differential; give caregivers the cloudy-eye guide and one-eye squinting guide.
Frequently Asked Questions
Does minimal tearing make a corneal ulcer unlikely?
No. Guinea pigs may have limited reflex tearing and can under-display ocular pain; stain and examine the cornea directly.
When should an ulcer be cultured?
Culture deep, stromally infiltrated, melting, recurrent, atypical, or treatment-unresponsive lesions, ideally before antimicrobial exposure.
Can tonometry be trusted through a diseased cornea?
Interpret cautiously. Edema, ulceration, corneal thickness, restraint, and device calibration can alter the reading; integrate it with the entire examination.
When is CT indicated?
Use CT for exophthalmos, suspected tooth-root or orbital disease, bone involvement, penetrating foreign material, facial swelling, or recurrent unexplained disease.
Should vitamin C be prescribed for every ulcer?
Confirm dietary adequacy and correct deficiency, but do not let supplementation replace diagnostics, antimicrobial decisions, analgesia, or structural treatment.

Deep stromal loss, malacia, descemetocele, perforation, uncontrolled infection, lens involvement, severe uveitis, glaucoma, or a threatened globe warrants urgent ophthalmic input.
