Paroxetine in Dogs and Cats
Bottom line
Paroxetine is a potent, highly selective serotonin-reuptake inhibitor (SSRI) used extralabel in dogs and cats for anxiety, fear and phobias, fear-based aggression, compulsive disorders, and feline urine marking. It is NOT FDA-approved for animals and has no veterinary formulation, so every use is off-label human product.[1] Typical maintenance dosing is 1-2 mg/kg PO q24h in dogs[1] and 0.5-1.5 mg/kg PO q24h in cats;[4] full behavioral benefit takes weeks, so it is a daily maintenance drug, not a situational anxiolytic.[2] In practice it serves as an SSRI alternative when fluoxetine is insufficiently effective or when fluoxetine's anorexia is a concern, but paroxetine carries its own clinically relevant anticholinergic burden — constipation and urinary retention — plus appetite suppression that matters most in cats.[3] Behavior modification remains the foundation; the drug is an adjunct.[2]
Drug facts
- Class: SSRI (selective serotonin-reuptake inhibitor).
- MOA: blocks presynaptic serotonin reuptake, raising synaptic serotonin in central serotonergic neurons.[2] Among SSRIs paroxetine is one of the most potent and selective SERT inhibitors; it also has more anticholinergic activity than fluoxetine or sertraline (a recognized property of paroxetine in human pharmacology), which clinically presents as constipation and difficulty urinating — both listed among its adverse effects by VCA.[3]
- Regulatory status: NOT FDA-approved in any species; the human product is used extralabel for every indication below, with no active veterinary formulation.[1]
- Canine indications (all extralabel): generalized anxiety, fear and phobias (storm and noise), separation anxiety, fear-based/conflict aggression, and compulsive disorders such as acral lick dermatitis and tail-chasing.[1]
- Feline indications (all extralabel): anxiety and excessive fearfulness, urine marking and inappropriate elimination, aggression, and compulsive disorders such as overgrooming/psychogenic alopecia and wool-sucking.[2]
- Dose: dogs 1-2 mg/kg PO q24h;[1] cats 0.5-1.5 mg/kg PO q24h.[4]
- Onset: 7-30 days to begin working, full effect over several weeks; short-term or situational use is inappropriate.[2]
- Key cautions: serotonin syndrome with other serotonergics or MAO inhibitors; hepatic impairment; seizure history; taper on discontinuation.[3]
Efficacy and indications
Prescribe paroxetine understanding that its veterinary support is largely extrapolated from fluoxetine and human data rather than from paroxetine-specific controlled trials. The Merck Veterinary Manual lists fluoxetine, sertraline, and paroxetine together for owner-directed aggression, fearful behaviors, obsessive-compulsive behaviors, and urine marking.[2] In practice, fluoxetine is generally used first-line and paroxetine reserved as an SSRI alternative — a clinical convention based on paroxetine's more anticholinergic, more sedating profile, not a specific Merck recommendation. The canine behavior drug table gives paroxetine for anxiety disorders, phobias, conflict and fear aggression, compulsive disorders, and urine marking — all as extralabel use.[1]
The best-quality SSRI evidence in this space is for fluoxetine, not paroxetine. In a placebo-controlled feline urine-spraying trial of 17 neutered cats, weekly spraying fell from a baseline of 8.6 episodes to 1.7 by week 2 and 0.4 by weeks 7-8 on fluoxetine, versus minimal change on placebo; reduced food intake occurred in 4 of 9 treated cats.[5] A meta-analysis of feline urine-spraying treatments found fluoxetine (odds ratio 13.36, 95% CI 3.43-52.06) and clomipramine (OR 4.23, 95% CI 2.00-8.93) significantly associated with at least a 90% reduction or cessation of marking; paroxetine was not among the treatments evaluated.[6] In practice paroxetine is used as a class substitute — pharmacologically reasonable, but the honest framing for colleagues and clients is that direct paroxetine trial data are sparse.
Adverse effects
The most common effects are sedation/lethargy, decreased appetite, and GI upset such as vomiting and diarrhea.[3] Because paroxetine is the more anticholinergic SSRI (a recognized human-pharmacology property), watch specifically for constipation and difficulty urinating; both are listed among its adverse effects and can be clinically significant, particularly in cats.[3] Decreased appetite deserves special vigilance in cats: anorexia occurred in nearly half of treated cats in the fluoxetine spraying study,[5] and a cat that stops eating is at risk of hepatic lipidosis — a reason to monitor food intake closely and intervene early. Other reported effects include tremor, restlessness or sleeplessness, panting, hypersalivation, and early paradoxical irritability or anxiety;[3] seizures have also been reported with SSRIs.[2] Serious reactions warranting reassessment include seizures, hyperexcitability, aggression, and persistent refusal to eat.[3]
Contraindications and drug interactions
The release-blocking interaction is serotonin syndrome, whose signs include altered mental status, agitation, tremors, myoclonus, hyperreflexia, hyperthermia, diarrhea, and cardiovascular changes.[7] Risk rises when paroxetine is combined with other serotonergic drugs or with MAO inhibitors: selegiline (Anipryl/l-deprenyl),[4] amitraz-containing products such as some tick collars, tramadol, tricyclic antidepressants (clomipramine, amitriptyline), trazodone, buspirone, and other SSRIs.[3] Do not co-administer with an MAO inhibitor, and observe an adequate washout when switching agents. Paroxetine is contraindicated in animals hypersensitive to it or to other SSRIs.[3]
SSRIs inhibit hepatic cytochrome P450 enzymes, so pharmacokinetic interactions with co-medications are plausible;[2] in humans paroxetine is specifically a potent CYP2D6 inhibitor, a flag when stacking it with CYP2D6 substrates. Use cautiously in patients with hepatic impairment, a seizure history, diabetes, or severe organ disease.[3]
Dosing and administration
Dogs: 1-2 mg/kg PO q24h (Merck canine behavior drug table; extralabel).[1] Cats: 0.5-1.5 mg/kg PO q24h.[4] Note that Merck's feline behavior guidance lists paroxetine only as an SSRI that can be considered for urine marking and gives no feline mg/kg figure,[2] and published feline figures vary (some sources cite 0.5-1 mg/kg q24h), so confirm the exact per-cat dose in Plumb's Veterinary Drug Handbook before dispensing and start at the low end. Give once daily and allow several weeks before judging response. Do NOT stop abruptly after long-term use — taper to avoid SSRI discontinuation signs.[3]
Monitoring
Obtain a baseline physical exam; consider CBC/biochemistry at baseline and periodically with chronic use, and in patients with hepatic concerns. Track body weight and appetite at every recheck, especially in cats, given the anorexia-to-lipidosis risk.[5] Reassess the behavioral response at roughly 4 weeks and again as the effect matures.[2] Because of paroxetine's anticholinergic activity, ask owners to watch litter-box and defecation habits for constipation or urinary retention.[3]
Alternatives
