Betta Swim Bladder Problems: Floating, Sinking, and Listing to One Side

Is "Swim Bladder Disease" Actually a Disease?
No. A betta that floats, sinks, tips onto its side or hangs nose-down and tail-up has a buoyancy problem — "swim bladder disease" describes what you are seeing, not what is wrong. Aquatic veterinarians are explicit that it is not a disease [4]; proposed causes run from constipation from over-feeding to infectious agents [5]. The swim bladder is an internal gas-filled organ that lets bony fish control buoyancy and hold their depth without wasting energy swimming [5]. Most advice online is written about goldfish, and bettas differ enough that much of it does not transfer.
Why Are Bettas Different From Goldfish Here?
The plumbing is opposite. Goldfish and koi are physostomous — a pneumatic duct connects the esophagus to the swim bladder, so they inflate it by swallowing air. Bettas are considered physoclistous: the swim bladder is not connected to the esophagus and fills using a rete system that pulls gas out of the bloodstream [2]. So the classic goldfish story — gulping air with floating flakes and over-inflating — does not map cleanly onto a betta. Sinking food is what that practice recommends, but its stated reason — cutting surface air intake [4] — is a physostome mechanism that does not apply to a betta, but for a betta the reason is portion control.
Bettas breathe air. They have a labyrinth organ: paired chambers above the gills, extensions of the first gill arch, containing folded bony structures covered with respiratory epithelium, which allow them to breathe air [1]. Be precise here, because the internet usually is not: Betta splendens is a facultative air breather, not an obligate one, unlike the blue gourami [3]. Your betta will not suffocate the moment it cannot surface. But the labyrinth organ is the site of aerial respiration in juveniles and adults [3], so a fish sunk on the bottom and too weak to swim up has lost a route it relies on — usually in a tank that is warm, unfiltered and low in oxygen.
What Usually Causes This in a Betta?
Food and water, far more often than infection.
- Constipation from overfeeding. A veterinary pharmacists' newsletter states buoyancy problems are normally caused by intestinal parasites or by constipation from over-feeding [5], though it cites no evidence for that.
- Cold water. Fish are ectotherms, so metabolism is tied to water temperature; if the water is too cold a betta cannot digest its diet, whatever that diet is [6].
- Poor water quality. Aquatic vets check this before anything else [4].
- Infection or organ disease. Swim bladder disorders can involve fluid accumulation, collapse, overinflation and herniation, with proposed causes including poor water quality, infectious agents, poor nutrition, neoplasia, injuries and genetic factors — a list drawn from koi, not bettas [10].
- The position itself does damage. A fish floating with skin exposed to air suffers disruption of the protective mucous barrier, which may lead to ulcerations and infections of the exposed skin and scales [5].
Show strains also carry finnage no wild betta has, and a heavy tail plausibly makes recovery harder — but we found no study measuring that.
Is It Dropsy Instead?
Check the scales, from above and head-on. Dropsy is coelomic distention due to ascites, with the scales standing erect from the body in a "pine-cone" appearance; it is a clinical presentation of disease rather than a disease entity [9]. A constipated betta looks fat; a pineconing betta looks fat and spiky, which is far worse. See dropsy in goldfish.
What Should I Do at Home First?
Test the water, fix the temperature, then fix the food.
Test the water. Check temperature, pH, ammonia, nitrites and nitrates [5]. An aquatic veterinary practice publishes these ranges for tropical fish: ammonia ≤ 0.1 mg/L, nitrite 0 mg/L, nitrate < 20 mg/L, pH 7.0–8.5, temperature 78–82°F (25–28°C) [7]. The MSD Veterinary Manual's freshwater table agrees on nitrite 0 mg/L and nitrate < 20 mg/L [8].
Get the temperature right, with a heater. University of Florida IFAS notes bettas thrive in warmer water and do not do well in cold water, and that in an unfiltered, unheated container the room's air temperature should be no less than 22°C (71.6°F) [1] — a floor, not a target. In the wild their water averages about 29.9°C, ranging from approximately 27.0 to 31.5°C [1].
Get out of the bowl. A board-certified fish veterinarian is blunt: if your betta is in an antiquated bowl, or worse a vase, move them to a heated, filtered tank as soon as possible [2].
Help them rest and breathe. For a fish stuck floating, bubble wrap on the surface protects exposed skin, and gravel is best avoided for permanently affected fish [4]. Keeping an affected fish off the bottom reduces infection risk, though the slings and floats used are imperfect [5].
Correct the portion. Feed enough to take up roughly the same space as one fish eyeball — anywhere from 10–12 pellets to 1–2 depending on your brand — with treats no more than once a week [6].
On fasting: we could not find a veterinary source specifying a fasting interval for a constipated fish, so this article does not invent one.
Do the Pea and Epsom Salt Remedies Actually Work?
The pea has some veterinary backing but a shaky mechanism. Epsom salt does not.
The pea. An aquatic veterinarian does suggest green peas — but explicitly not for their fibre: that source states plainly that fish cannot digest fibre, and that the peas are simply healthy and they sink [4]. Sinking is the only mechanism it offers. A veterinary pharmacists' newsletter calls a green pea "a simple remedy that can work within hours" — but writes that scientists assume it works by countering constipation [5]. Bettas are carnivorous rather than omnivorous like goldfish, which is our own reason for doubting that mechanism — we found no study testing peas in bettas. A small piece of pea is unlikely to hurt — blanching and deshelling are common hobbyist practice, not something the veterinary source specifies — but do not treat it as a cure.
Epsom salt. Hobbyists recommend it constantly and we could not find veterinary support for it. Epsom salt is magnesium sulfate; it does not share the beneficial properties of sodium chloride and should not be used as a substitute, and while it does not hurt fish, it is not good for them at high or long doses [12]. This article therefore gives no dose.
Know the ceiling: a veterinary review of buoyancy disorders in ornamental fish — a caseload dominated by goldfish, not bettas — found a very poor response to treatment [13]. Many fish do still live long, happy lives with permanent buoyancy issues when the tank is managed carefully [4].
How Do I Stop This Happening Again?
- Heated, filtered tank — not a bowl, not a vase [2].
- More space, furnished. In a peer-reviewed welfare study, bettas in a 19.3 L tank swam significantly more than in a 1.5 L jar, a 3.3 L small tank, a 5.6 L medium tank or a barren 19.3 L tank, with more swimming and foraging and less stereotypic pacing and hovering [11].
- One eyeball-sized portion per feeding — the source gives a portion, not a frequency [6]. Many keepers pre-soak dried pellets so they swell in the cup rather than in the fish — sensible, but we found no veterinary sourcing for a soak time.
- Replace fish food every 6 months, as water-soluble vitamins including vitamin C degrade [6].

When to See a Vet
Aquatic veterinarians exist and are findable — search for a fish or exotics practice near you.
- Scales standing out in a pine-cone pattern with a swollen belly, pointing to ascites and serious underlying disease rather than constipation [9].
- Your betta cannot reach the surface at all, or lies on the bottom unable to right itself — the labyrinth organ is its normal air-breathing route [3].
- The problem persists after temperature and water quality have been corrected and held in range (no veterinary source specifies how long to wait before referral).
- Raw patches or ulcers where the fish has floated with its body exposed to air [5].
X-rays are the most useful method for visualising how the swim bladder is distributing gas, and can indicate over-inflation, displacement secondary to a tumour or mass effect, rupture, or severe gastrointestinal problems [5]. If bacteria are present the appropriate antibiotic can be used; if there are none, antibiotics should be avoided [5].

Why is my betta floating at the top and unable to swim down?
That is positive buoyancy. A constipated betta has a rounded belly with the scales lying flat.
Do bettas have to reach the surface to breathe?
Not strictly, but they rely on it. University of Florida IFAS describes the betta's labyrinth organ as paired chambers above the gills that allow them to breathe air. Research on air-breathing anabantid fishes classifies Betta splendens as a facultative air breather, unlike the obligate air-breathing blue gourami, and identifies the labyrinth organ as the site of aerial respiration in juveniles and adults.
